Please Note: The HR Smart Forms on this page are for the following agencies using HR21: Department of Human Services, Department for Child Protection, EECSRSB, South Australian Housing Authority and Renewal SA.

In line with the migration to CHRIS 21, ‘Smart’ HR forms have been created. These Smart HR Forms replace 30+ existing paper-based forms.

The functionality includes the pre-population of various fields from the details held in CHRIS 21 reducing the amount of input required.

The forms are accessible via the Shared Services SA website:
